Enrolling in Certificate Maintenance

Important Information Regarding Changes in the Recertification and CMP

• One module per each of the three areas of practice is available at no charge. A module includes: test booklet, answer sheet, and return envelope.
  
• The annual activity fee for the Certificate Maintenance Program is $90.00 and is subject to change without notice to those who pay on an annual basis.
  
• The late fee, if the Annual CMP Activity Report form and fee are received at AMCB Headquarters past the May 31st deadline, is $50.00.
• If you would like to pay the entire 5-year fee the first year of your cycle, the fee is $292.50 and is subject to change. (January 2010)
• The fee for all article packets is $95.00. If you have access to the articles by other means, there is no need to order this packet.
  
• Duplicate modules may be purchased for $25.00.
  
• Duplicate answer sheets will be mailed upon request.
